The acute care of frail older people has changed dramatically over the last decade. Where previously both acute and sub-acute care was delivered primarily in hospital settings, sub-acute care is now being provided in a variety of community settings and by a range of professionals.

The aim of this one day conference is to bring together acute care physicians, geriatricians (including community geriatricians) and other interested parties to examine different models of delivering comprehensive geriatric management across primary and secondary care.
