The Reablement Service is a grouping of inpatient and outpatient services which includes Older People’s Health and Rehabilitation for younger adults. It is part of the Community and Long term Conditions Directorate of Auckland District Health Board (DHB). The purpose of the directorate is to provide quality, patient-centred, self-directed care as close to home as possible by working in partnership, enabling self-management, and promoting independence. The Reablement service aims to provide world class assessment, treatment, and rehabilitation to adult patients to optimise their health, function, and participation.

An opportunity exists for a Geriatrician to join the Reablement Service at Auckland District Health Board.

As a Geriatrician you will be part of a team providing assessment, treatment and rehabilitation services to older people in inpatient, outpatient and community settings. There are exciting developments in the community with moves to an integrated locality model supported by a range of new transitional care services.

This is an exciting time of service redesign to provide quality, patient centred, self-directed care as close to home as possible.
