NICE guideline on transition between inpatient hospital settings and community or care home settings

NICE has published a new resource to support the implementation of the guideline on transition between inpatient hospital settings and community or care home settings for adults with social care needs. It was developed by the NICE Collaborating Centre for Social Care (NCCSC), a partnership led by SCIE.  

Called "Improving how community and hospital-based staff work together to ensure coordinated, person-centred support", the resource has practical information on how to improve support for people as they transfer from their home to hospital and back again. It is based on the experiences of health and social care professionals and people who use services in two areas of the country – Torbay and Derbyshire.
