Delivery of healthcare is changing as the 10 Year Health Plan is implemented in England and healthcare services look to introduce the three shifts from hospital to community, sickness to prevention and analogue to digital. At the BGS, we think it is also time for systems to consider how they can be frailty and age-attuned, ensuring that they are able to provide high-quality care for older people, when and where it is needed.
Our Integrated Systems project brings together BGS members from different care settings and professions across the country. They have helped to compile resources to support people who want to create frailty and age-attuned systems. Through this project we are sharing top tips, template job descriptions, video conversations between those who have been there and those who are learning, and blog posts sharing experiences. This is not an exhaustive set of resources – we will have more to add over the coming weeks. The aim is to foster learning between different systems and avoid needless reinvention of the wheel.
We encourage you to take advantage of what others have learned as you develop your own system to provide joined-up person-centred care for older people.
