Advance Care Planning: A Patient Journey

Course Overview

This short CPD course follows the journey of a patient as her health deteriorates and the interactions she has with different healthcare professionals in different settings.

As well as the 4-part case study videos, there are also some reflective questions to answer. This will help you to complete an advance care plan that is patient-centred and contains the right information.

  • To reflect on how we can provide good advance planning to older people living with frailty
  • To reflect on how we use ACP in our own care setting and what would help us to do it better
  • To think about where ACP sits within a person’s life

How to access this microlearning

Once registered, you can immediately access the course. If you wish to come back and take the course later, you can do so by logging into your BGS website account.
  • Click on 'My account' at the top right corner of the page and then select the 'CPD and certificates' button (on the right hand side).
