Living and dying well with frailty - Quality end of life care for older people

High quality care for the older person living with multiple conditions and frailty must involve affirming life and promoting independence but also recognising and planning for death. This conference aimed to bridge these gaps, get professionals talking about their roles and listening to each other so that together we can improve services and care. We have kindly received permission from some of our speakers to reproduce their presentations. 

Dr Adrian Hopper, Consultant Geriatrician Guy's and St Thomas' Trust

Adrian Hopper is a Geriatrician with a particular interest in patient safety, quality of care and spread of innovation. He was part of a small team that developed the AMBERcarebundle to improve reliability of patient centered decision making in patients who have uncertain recovery and has helped it spread widely in UK and internationally.

Dr Martin J Vernon, Consultant Geriatrician, National Clinical Director for Older People, NHS England and Dr Dawn Moody, Associate National Clinical Director for Older People and Integrated Person Centred Care, NHS England

Dr Martin Vernon is a Consultant Geriatrician in Manchester and National Clinical Director for Older People and Person Centred Integrated Care at NHS England. He is also Clinical Chair of the Hospital to Home Programme Board, one of six programmes within the Urgent and Emergency Care (UEC) Delivery Programme, led and governed by NHS England and NHS Improvement.

Dr Dawn Moody is Associate National Clinical Director for Older People and Integrated Person-Centred Care for NHS England Dawn is a GP with a special interest in the care of older people, working in Derbyshire and Cheshire. She has worked extensively with commissioners, providers and in research in this area. Dawn has an MPhil for her frailty research and holds an MSc in Geriatric Medicine. Dawn’s current portfolio also includes a teaching role at Keele University and her directorship of Fusion48, which includes her role as Clinical Director for the HEEM Nottinghamshire Frailty Toolkit and Training Programme.

Dr Caroline Nicholson, Senior Clinical Lecturer Supportive and End of Life Care Nursing, King's College London and St Christopher's Hospice

Caroline is a Senior Clinical Lecturer working across the Florence Nightingale Faculty of Nursing and Midwifery, King’s College London and St Christopher’s Hospice. This unique partnership allows her to research and contribute
to the strategic development and clinical management of supportive and end of life care services for older people living and dying with advanced frailty /multiple morbidities. Caroline has worked in the NHS for over 30 years, both directly as a Clinical Nurse Specialist in Palliative Care, and more recently as a nurse academic. Her research programme focuses on the intersection between gerontological and end of life care, as well as the capacities and frailties of people living in late old age. Caroline’s work aims to enhance the development of care systems that recognise, facilitate and promote dignified and supportive care for older people and their families at end of life.

Dr Adrian Treloar, Consultant and Senior Lecturer in Old Age Psychiatry, Oxleas NHS Foundation Trust

Dr Adrian Treloar, FRCP, MRCPsych, MRCGP is a Consultant and Senior Lecturer in Old Age Psychiatry in South East London and was formerly a GP.
