Virtual Wards for Older People Living with Frailty: A Review of Effectiveness
Abstract
Introduction:
Virtual wards have been increasingly implemented across the NHS to manage older people living with frailty in their own homes amid rising inpatient pressures. As this care model expands, it’s essential to determine whether virtual wards yield outcomes comparable to those of traditional inpatient care. This review aims to assess the success of virtual wards across key outcome domains.
Methods:
A literature review of ten studies published between 2014 and 2025 was conducted using electronic databases with relevant keywords. Studies examined multiple aspects of virtual wards, including costs, patient outcomes, and stakeholder perspectives.
Results:
Many studies support virtual wards for frail older people with shorter lengths of stay than inpatient care (4 vs. 14 days, p < 0.001)¹ and a patient preference for care at home.² However, higher mortality at 30 days, 3 months, and 6 months, alongside an 11% absolute increase in readmissions, raises concerns about long-term effectiveness.¹ Evidence on cost-effectiveness is mixed, with some studies reporting savings of over £3,000², while others remain inconclusive.
Conclusions:
Overall, virtual wards represent a reasonable alternative to inpatient frailty care, but variable outcomes highlight the need for clearer patient selection, workforce training, and standardised pathways. Staff transition issues reveal gaps in care pathways that risk widening regional disparities. Whether virtual wards can be maintained beyond specialist teams remains uncertain; a greater multidisciplinary understanding is essential for wider NHS roll-out
