Action Falls Lead (e-learning) Training: An Evaluation using the Kirkpatrick model
Abstract
Introduction:
The Action Falls programme is a multifactorial falls-prevention programme shown in a randomised controlled trial (RCT) to reduce falls in care home residents by 43%. Action Falls has since been recommended as best practice by NHS England. In the original RCT, training was delivered by a member of the research team face-to-face to “Falls Leads” (registered NHS nurses or allied health professionals), using a train-the-trainer model. A subsequent implementation study indicated that an e-learning approach could offer greater flexibility and scalability. This study evaluated Falls Leads’ experiences of completing the Action Falls e-learning programme and then delivering training (face-to-face) to care home staff.
Methods:
The original training was adapted into an e-learning course, supplemented by optional webinars and CPD accreditation. Falls Leads (registered nurses, allied health professionals, and suitably qualified social care staff) received access to digital and printed resources. Purposeful sampling aimed to recruit ten Falls Leads plus two individuals involved in commissioning decisions to take part in semi-structured interviews on completion of the e -training. Data were analysed using Reflexive Thematic Analysis.
Results:
Nine interviews were completed. Data analysis was mapped to the Kirkpatrick model. At Level 1 (Reaction), Falls Leads reported acceptability, finding the e-learning flexible and relevant, some experienced navigation challenges and concerns about training time. Level 2 (Learning) demonstrated consolidation of existing knowledge, and engaging care home staff. Level 3 (Behaviour) showed variable implementation of training, influenced by staffing pressures, difficulties integrating the paper-based checklist with digital systems, and the need for care home managerial support. Level 4 (Results) reflected perceived benefits and commissioning interest but highlighted constraints and challenges.
Conclusion:
E-learning is an acceptable method for delivering Action Falls training. However further research is required to explore how the training model can be adapted to meet the needs of care home staff.
