The BGS has published Integrated Systems: Practical resources for age-attuned care, a collection of materials to support those keen to implement frailty and age-attuned services.
Our Integrated Systems project brings together BGS members from different care settings and professions across the country. They have helped to compile resources to support people who want to create frailty and age-attuned systems. These include top tips, template job descriptions, video conversations between those who have been there and those who are learning, and blog posts sharing experiences. This is not an exhaustive set of resources – we will have more to add over the coming weeks. The aim is to foster learning between different systems and avoid needless reinvention of the wheel.
To provide care of the highest quality for older people when and where they need it, we believe healthcare systems should consider how they can be frailty- and age-attuned.
We encourage you to take advantage of what others have learned as you develop your own system to provide joined-up person-centred care for older people.
