We have just published a new resource in our Integrated systems: Practical resources for age-attuned care series. Integrated systems: Population health management outlines key considerations to guide systems wishing to use Population Health Management (PHM) approaches to support effective, frailty-attuned proactive and personalised care. It aligns with the three shifts as set out in the Ten Year Health Plan for England, although the themes will be relevant to colleagues from across the four nations. The document provides top tips for using a PHM approach and case studies of areas successfully using these methods.
